Complete New Patient Forms

Arrive 10–15 minutes early to complete your health history and intake forms, or ask about completing them in advance.

Initial Evaluation

Dr. Lewis will perform a thorough clinical examination, review your history and any imaging, and discuss your symptoms in detail.

Diagnosis & Care Plan

You'll receive a clear diagnosis and a specific care plan with defined goals — no vague timelines or unnecessary visits.

What to Bring

  • Valid photo ID

  • List of current medications and supplements
  • Prior medical records related to your condition (if available)
  • Completed new patient intake forms (if sent in advance)

Do I need a referral to see Dr. Lewis?

Most plans do not, however some HMO plans will require a referral — call the office to verify before your visit and we'll help you sort it out.

What should I bring to my first appointment?

Please bring a valid photo ID, your insurance card, any relevant imaging (X-rays, MRI, CT scans), and a list of current medications. Arriving 10–15 minutes early to complete new patient paperwork is recommended.

Does Dr. Lewis accept insurance?

How long is the first visit?

The initial evaluation typically takes 45–60 minutes. This includes a thorough health history review, physical and orthopedic examination, and a discussion of your diagnosis and recommended care plan.

How many visits will I need?

That depends entirely on your diagnosis, the severity of your condition, and how your body responds to care. Dr. Lewis will give you a clear, honest estimate after your initial evaluation — no open-ended treatment plans.

Is chiropractic care safe?

Yes. Chiropractic care is one of the safest forms of treatment for musculoskeletal conditions. Dr. Lewis performs a thorough clinical assessment before any treatment to ensure the approach is appropriate for your specific condition.

Do you treat auto accident injuries?

Yes. Dr. Lewis has extensive experience treating whiplash, soft tissue injuries, and spinal trauma resulting from auto accidents. The office works with personal injury attorneys and insurance carriers and provides thorough medical documentation.

What if I've had back surgery — can I still be treated?

In many cases, yes. Dr. Lewis will review your surgical history and imaging before recommending any treatment. Some post-surgical patients are excellent candidates for chiropractic care; others may be better served by co-management with their surgeon.
